Document Retrieval Price List
Price list for retrieving and copying of medical, employment, insurance, academic and public records. These prices are for AUTHORIZATION ORDERS ONLY.
Each base price includes for each order, delivering a request to copy records to the custodian of records, set up for photocopying all documents, 3 field trips, phone calls to the location, and mileage. Additional charges for mileage will be applied to orders if minimum requirements are not met (see price list below). The price per page is not included in the base price. Each SUBPOENA SERVICE includes, serving all documentation to the location on the subpoena, set up for photocopying all documents, field trips, phone calls to the locations and mileage.
